CIBHS helped California Small County Whole Person Care Collaborative program staff use data to capture their program outcomes and lessons learned from their pilot program. Participants increased engagement in primary care, decreased use of emergency departments and stays in hospitals, and improved mental health outcomes.
CIBHS helped WEAVE implement the Strengths Model as a solution to enhance their case management services and to support survivors in achieving their personal goals. The model has had a positive impact in providing the structure, tools, and skills to enable people to make progress in their life.
CIBHS designed a custom 12-week virtual training series that offered clinical techniques, tools, and resources to mitigate implicit bias, and to help with engagement, crisis intervention, harm reduction, and trauma-informed interventions when providing telehealth.
CIBHS provided capacity-building training to guide LA County SAPC through the transformation process required by the DMC ODS waiver. Providers were able to successful apply change management tools to improve recovery and wellness for people receiving services as well as to achieve financial success for the organization.
